Next, we will review your completed intake forms and reach out to you if there are any issues. Once our office confirms your appointment request, you will receive a confirmation email from firstname.lastname@example.org.
Send us a photo of your insurance card (front & back) and your photo ID
Our office requires a photo copy of front and back side of your insurance card, as well as a photo ID. Email a photo of all three to email@example.com, or create an account with Practice Better and upload into the Documents section.
Please provide these documents at least 48 hours before your appointment, or your appointment will be cancelled.
We recommend verifying your insurance benefits for nutrition counseling.
Please use this link if you would like to verify your own benefits by calling your insurance company. If you would like us to verify your benefits for you, please email us at firstname.lastname@example.org with “Verify My Benefits” in the subject line after booking your appointment. The charge is $15 for us to verify your benefits on your behalf.
Verifying your nutrition benefits helps to reduce the risk of surprise medical bills from our office. Please note that verifying your benefits does not guarantee coverage, as insurance company representatives at times misquote benefits, however verifying benefits greatly reduces the chances of surprise bills.
